The state Division of Occupational Safety and Health, along with inspectors, discovered that machinery in the meat grinding room was not properly closed at the time of cleaning.
Consequently, they ruled that an unexpected start-up of the machine could have been the cause of this personality’s disappearance.
Interstate Meat Distributors President Darrin Hoy shared that company officials fully cooperated with investigators. Additionally, the factory was also the subject of a consumer alert in 2007.
Back then, authorities traced the potentially deadly E.coli bacteria to ground beef from this factory. An accident occurred at the plant in early 2005, as the Oregonian reports.
