After completing his formal education, Afzal started looking after his family business. However, dissatisfied with the work, he decided to leave it and relocate to Mumbai to pursue a career in acting. In 1999, he appeared in the Bollywood film Tower House. In 2001, he made his debut in the Hindi entertainment industry with the Hindi daily soap opera Kyunki Saas Bhi Kabhi Bahu Thi produced by Ekta Kapoor, in which he played the role of a spoiled brat. He talked about this in an interview and said,
I came to Mumbai and started looking for some good work and I came to know that auditions were going on for some show called Because Saas Bhi Kabhi Bahu Thi and my token number was 701, and they were looking for a character and I was He got it. The show and the character was bad, negative character and I was told it was an 8-10 day character and I took that job.
The same year, Afzal Khan got a role in the Zee TV serial Kitne Kool Hain Hum, produced by Ekta Kapoor. According to Afzal, she was offered a role in the serial by Ekta as she was impressed by her acting skills in Kyunki Saas Bhi Kabhi Bahu Thi. In 2003, Afzal got a role in a Hindi film titled Kaash… Hamara Dil Pagal Na Hota. In 2006, she got a role in the Hindi soap opera Sanskaar.
