After completing MBBS, Sana passed the FCPS-1 (Fellow of College of Physicians and Surgeons) examination, after which she began her training to become a Professional Fellow at the College of Physicians and Surgeons (CPSP), Pakistan. He did his training in various medical colleges including Chandaka Medical College Larkana, Sindh and Sindh Institute of Urology and Transplantation Karachi, Pakistan. When she passed FPSC CSS 2020, she was working as a Grade 17 Medical Officer at Taluka Hospital, Lakhi in rural Shikarpur.

Sana Gulwani started her preparation in May 2019. During his training as a surgeon, he experienced poor management and facilities in hospitals. Being a businesswoman, she could not do much to improve the situation. In an interview he talked about the incident which became the turning point of his life. He said,

I was not even aware of CSS until District Commissioner Shikarpur visited our hospital. I was impressed by his conduct and his communication skills. Later, I learned that he was a CSS officer, which became a turning point for me. It took me about two months to do thorough research and make up my mind to sit for the exam. I started my preparation in May of 2019 and appeared for the exam in February 2020.”

He managed his medical training and CSS studies simultaneously. Initially, she studied only 3-4 hours a day due to her training and thesis work. In November 2019, he took leave from his job and started studying 12-14 hours a day. In an interview, he shared his thoughts about CSS and said,

I think we create unnecessary fear of CSS in our minds. This is just a hype that people have created. I personally experienced that CSS is easier than therapy. We can say that it is complicated. If someone understands this trick then he can pass the exam like me with just 7-8 months of preparation.

FPSC CSS has six compulsory subjects and six optional subjects. Sana chose International Relations, United States History, Zoology, Sindhi Literature, Town Planning and Urban Management and Applied Mathematics as her optional subjects. She deactivated all her social media accounts and cut off contact with friends to prepare dedicatedly. He took the help of his MBBS batchmate who was also preparing for civil services.

Unlike other candidates, Sana chose to stay in Shikarpur rather than going to Lahore or Islamabad. He got the support of many people including his seniors and family. He took guidance from a senior working in Income Tax; He helped her choose optional subjects, checked her essays and compositions and guided her whenever she needed help. Sana’s family supported her in every possible way. Speaking about his mother’s efforts in a talk show, he said,

My mother took the utmost care of me during the entire exam preparation phase. Since I had been discharged from the hospital, I never left my room during the last three months before the exam. If I read all night, she would stay up with me; I didn’t even have to get up for tea. He took care of everything not only during my CSS preparation but also during all my exams. I have always had him in my corner.”

On 6 May 2021, FPSC declared the result after which Sana Gulwani went to Civil Services Academy Lahore for Common Training Program (CTP) in 2021 and Special Training Program (STP) in 2022.
