In 2014, he represented India in a cultural exchange program held in Japan. On 25 June 2017, she participated in Femina Miss India 2017 and represented the state of Haryana. During the competition, she was also crowned Miss Photogenic during the sub-competitions.

On 18 November 2017, she won the 54th Miss World 2017 pageant and was crowned Miss World 2016 by Stephanie del Valle of Puerto Rico.

Manushi left medical school to pursue a career in modeling and acting. When asked about his decision he said,
After winning Miss World, I worked really hard to pass my medical entrance exam in college. I always wanted to be a doctor, I wanted to use the experience I gained as Miss World and combine that with my medical degree. But life had other plans. I never thought that I would take up acting as a profession.

Manushi Chhillar returned to India on 26 November after becoming Miss World 2017, where she did many works related to charity and public welfare. Chhillar’s first press conference as Miss World was at the St. Regis Hotel in Mumbai. In December 2017, the Government of Haryana named him the brand ambassador for the Anemia Mukt Haryana (Anemia Mukt Haryana) project.

Her project called ‘Project Shakti’ received funding of Rs 18 crore from the government to distribute free sanitary napkins to all girls in government schools in Haryana. Under this project, she visited 20 villages and helped about 5000 women. She also educated women about HIV/AIDS. She was on Set India’s Super Nights with Padman to talk about her project on menstrual hygiene with Akshay Kumar’s film Padman.

In February of 2018, she along with other Miss World titleholders launched her Beauty with a Purpose Tour, traveling to Hyderabad and Kolkata to advocate for feminine hygiene awareness. Joining an NGO in Kolkata, she acquired the skills to make affordable sanitary pads and worked to ensure their availability in rural areas. After covering Hyderabad, Kolkata, Mumbai and Delhi during her 42-day tour in India, she visited Hong Kong and China and extended the tour to Indonesia.

In Indonesia, she watched a local project and was crowned Miss Indonesia 2018. Additionally, in 2018, she contributed to a charity initiative in the British Virgin Islands and participated in ‘Beauty with a Purpose’ as well as a gala dinner and auction. In May 2018, Manushi walked with many women in Delhi to tell people how important it is to manage periods well. He also helped launch a big bike ride in Delhi to celebrate World Bicycle Day. Manushi worked with UNICEF to educate people about the coronavirus pandemic and stay safe. She campaigned for governments to provide sanitary pads to female workers during the lockdown in India. In 2021, she joined with UN Women to speak out against violence against women. She also supports a project called ‘Orange the World’ and advocates stopping online harassment of women.

Chillar signed a contract with Yash Raj Films to act in three films. Her first film was Samrat Prithviraj (2022), in which she played Sanyogita, the wife of Prithviraj, played by Akshay Kumar. Chillar said that it was a big responsibility to play Sanyogita, a strong character who takes her own important decisions. The film did not perform well at the box office and critics did not like her acting.

In 2023, she appeared in The Great Indian Family. After his role in this film, he also had to face criticism for his acting. In 2024, she starred in “Operation Valentine” alongside Varun Tej. - After winning Miss World, she was given the Special Achievement Award at the 2017 CNN-IBN (also known as CNN-News18) Indian of the Year Awards.
